During the 2011/12 period, Park End Surgery have been participating in a Patient Participation service. During this time we have engaged in further discussion with our patient group and conducted a patient survey by paper and e-mail.
The action plan for 2012/13 is:
Improve patient information and access to it
1) review and renew our practice website – already under construction
2) information folders for waiting areas
3) continue to use (and keep updated) our patient e-mail addresses and regularise our contact with this virtual group
4) Notice boards -using an area of notice boards for PRACTICE NEWS
Opening times
To continue to ensure that our practice population are fully aware of all the different clinics that we provide and the times of these to reflect the answers to our practice questionnaire of 2011/12.
The way the practice will achieve this is
- new edition to the practice information sheet
- handout in consulting rooms
- poster and leaflet as per information section above
- website updates
- email to virtual PPG
To address issues with our branch site as best possible regarding public transport. Dr McCann will undertake this on behalf of the practice patient group.
And to continue to meet with our Patient Reference Group and communicate with our Virtual Patient Group regularly.

The surgery is open as follows:
Bridgewater House & Meriden standard opening times 8am – 6.30pm every weekday
In addition to this
On Mondays – Bridgewater House extends its sessions to 7pm
On Tuesdays – Bridgewater House works extended hours until 8.30pm

https://systmonline.tpp-uk.com to complete the registration process and password protect it.
Summary Care Records - our data, including allergies, sensitivities and repeat prescriptions will be uploaded to the demographics already held by the Spine in January 2012. No other data will be uploaded at this time. We would like to reassure any patients who have opted out by writing to us, will NOT have any data uploaded.
Patient Participation - we have a patient participation group who meet regularly to have an input into the Practice. In addition to this we would like to invite patients to join our VIRTUAL PATIENT GROUP to answer short questionnaires or to respond to us by email.
